CONSULTATIONS
An appointment will comprise of:
New patients form to be filled out by the patient, followed by a thorough case history, assessment and treatment taken by your practitioner.
Treatment will be conducted if clinically indicated, patient consent will be obtained prior to any assessment or treatment procedures. If treatment is indicated your practitioner will explain the assessment and any techniques/exercise proposed plus any potential risks.
If after a case history, assessment and consultation, the patient is not a candidate for treatment, the patient may be recommended to further management by a relevant healthcare professional. In this event you will still be charged the full consultation fee for the appointment.
Payment is required at the time of your consultation. Rebates from private health insurance may be possible if you have the appropriate professions cover in your extras cover.
ONLINE BOOKING
By registering as a patient online you are subscribing to receiving emails from Keep Active Osteopathy. You can unsubscribe at any time.
You can cancel your appointment by clicking the cancellation link on your confirmation email as long as it is 24 hours prior to your appointment.
Please call the clinic to cancel or move your appointment if it’s less than 12 hours before your appointment.
ONLINE FORMS
All digital online forms are sent to your medical file held on our practice management system Pracsuite. The form/file is part of your medical file. For information regarding our privacy policy please refer to our website.
CANCELLATION POLICY
We have a strict 12- hour cancellation policy. By booking an appointment with us and consenting to our consent to treatment you agree to provide us with a minimum of 12-hours notice of cancelling or moving an appointment or a late cancellation fee up 50-100% of the scheduled fee with be charged.
We understand there can be situations outside of your control and we will do our best to have open communication with you before sending a cancellation fee.
All third party insurance patients such as TAC and Worksafe must personally pay the cancellation fee. The cancellation fee cannot be sent to the insurance provider.
If you have been sent a cancellation fee, you will not be eligible for treatment until the cancellation fee is paid.
THIRD PARTY PAYMENTS
Third party payments are payments made by organisations such as but not limited to:
WORKSAFE
Keep Active Osteopathy may charge a gap fee to Worksafe patients.
Patients must bring in all documentation for their Worksafe claim, all Worksafe claims will be billed upfront unless in extenuating circumstances (a patient may request a gap only claim).
All patients covered under Worksafe are required to attend regular appointments and complete pain / functional questionnaires at regular intervals.
Cancellation policy still applies.
TAC
Keep Active Osteopathy may charge a gap fee to TAC patients.
Patients must bring in all documentation for their TAC claim, we will need to confirm the claim is valid before billing TAC on the patients behalf. You may be required to pay upfront until confirmation is made.
All patients covered under TAC are required to attend regular appointments and complete pain / functional questionnaires at regular intervals.
Cancellation policy still applies.
MEDICARE / CHRONIC DISEASE MANAGEMENT
Chronic disease management (CDM) Medicare plans are for patients with a chronic illness and require ongoing care. You can be allocated up to 5 treatments per year and must be referred from a General Practitioner. Speak to your GP today to see if you’re eligible.
A full fee upfront payment is required and we will process your claim for a rebate which will be deposited directly into your bank account.
Cancellation policy still applies.
